East Care Urgent Care \u2013 Terms and Conditions<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

\n\t\t\t\t\t\t\t\t\tMedical Priority – Priority is given to patients with serious or potentially life-threatening conditions, otherwise patients are seen by the clinician in the order they arrive at East Care Urgent Care. To help determine your triage category please ensure you have advised the Triage Nurse the reason why you have come to be seen today. This will determine the severity and priority of your presenting symptoms.\n\nHealth Information Privacy Statement – A copy of East Care\u2019s Health Information Privacy Statement is available from Reception.\n\nConsultations Notes and Test Results – East Care sends clinical notes and test results to your nominated GP daily. If you would prefer that your notes and test results are NOT sent to your doctor, please ensure you have checked the \u201cDO NOT SEND\u201d box on your Encounter Form.\n\nConsultations and Payment – At East Care our clinicians include – doctors, nurse practitioners, registered nurse prescribers, registered nurses and paramedics. You may be seen by any one of these clinicians depending on your presenting complaint. Consultations fees apply to all consultation services performed by any of these clinicians carried out either in our clinic, in your car or by phone consultation.\u00a0 All unpaid invoices will be charged an administration fee of $15.00. All overdue invoices greater than one month will be passed to our external debt collector. All fees incurred in this process will be charged to the patient.\n\nCommunity Services Card (CSC) – Community Services Cards must be presented at the time of consultation, otherwise a full consultation fee will apply. Cards presented within one week (7 days) of the consult may be eligible for CSC rates.\n\nACC – If you are presenting for an accident, your costs today may be partially covered by ACC. You are responsible for the balance. Any claims not accepted by ACC will be invoiced to you. Please ensure you have completed the ACC forms required.\n\nAmbulance Transfers – Any fees incurred for ambulance transfers are the responsibility of the patient.\n\nLaboratory Tests – As East Care Urgent Care is not your primary health care provider, we will only be contacting you should your results indicate further follow-up is required. The quickest way for you to obtain your test results is through your GP as they are also able to interpret your results and provide follow up treatment if required.\n\nMedical Certificate – If you require a Medical Certificate, please ask at the time of consultation. If a Certificate is required at a later date, a consultation fee will apply.\n\nFeedback – We welcome your feedback to help us improve our service to you. Our comments box is located in the patient waiting area or you can email us on info@eastcare.co.nz\/a> or write to: The Clinical Director, P.O. Box 38-306, Howick 2145.\n\nFamily\/Whanau or other Support Persons – You are welcome to have a support person accompany you during your consultation. Please be aware that they may have access to sensitive patient\/doctor information discussed during your consultation.\n\nLocal GP – If you do not have a regular GP, please pick up a list of our local GPs from reception. For on-going medical advice and treatment, regular GP monitoring is always best.\n\nHDC – Health and Disability Commissioner (HDC) pamphlets on Patient\u2019s Code of Rights and The Consumer\u2019s Guide are available in the Waiting Room.
